SWSPHN is hosting an event at Tahmoor Community Centre to bring the community together to talk about palliative care and the advantages of Advance Care Planning during National Palliative Care Week, 19 to 25 May.

SWSPHN is hosting an event at Tahmoor Community Centre to bring the community together to talk about palliative care and the advantages of Advance Care Planning during National Palliative Care Week, 19 to 25 May. The event is on Thursday, 23 May, from 10am to 1pm. We are encouraging GPs and Practice Nurses to share this information with your patients.

The focus of this year’s awareness week is ‘what matters most’ and addresses the need to plan ahead for end-of-life care and discuss it with loved ones and health professionals. Resources are available – the online Dying to Talk Discussion Starter and Dying to Talk Card Game – to help work out what matters most to you.
