Don’t miss being paid for case conferences

GPs are reminded the following activities need to be completed to claim case conferencing item numbers:
- Obtain patient’s consent.
- Record meeting details including time, date, location, attendees, relevant medical matters and outcomes.
- Discuss outcomes with patient and offer a copy of the document.
- Can be conducted face-to-face, via the telephone or videoconference link.
A case conference must involve at least three health or community care providers including the GP, one of whom may be another medical practitioner. Each should provide a different type of care or service to the patient.
Examples include but are not limited to: Aboriginal health care workers, audiologists, dentists, diabetes educators, dietitians, occupational therapists, optometrists, pharmacists, physiotherapists, podiatrists, psychologists, nurses, social workers, home and community service providers such as personal care workers.
The patient and their carer may be included in addition to the minimum three attendees.
Discharge case conferences may be provided for private in-patients being discharged from hospital into the community.
