GPs invited to take Transition Model of Care survey

South Western Sydney Local Health District (SWSLHD) is developing a medically–led model of care to support the transition of young people with chronic complex conditions from paediatric to adult services, and feedback from General Practice teams and paediatricians is fundamental to this process.   

The transition from specialist paediatric medical services to adult services is challenging for any family, even more so when a young person has chronic complex conditions with intellectual/developmental disability. Transition care is “the purposeful, planned movement of adolescents and young adults with chronic physical and medical conditions from child-centred to adult-oriented health care systems” (Blum et al., 1993, p. 570). Although it is known that general practitioners play a significant role in the care of these young people, there is no significant research on GP led transition models (Bhawra et al., 2016).
