New mental health program launch

South Western Sydney PHN is excited to launch Connect for Wellness, a ground-breaking new program for people living with severe and persistent mental illness. The service provides flexible psychological support through three integrated components - psychological intervention with a mental health professional (unlimited within reason), group therapy and engagement with a Connect for Wellness Peer Worker as a vital and consistent form of support throughout the program.
The service is available for people in South Western Sydney living with severe and persistent mental health issues, who are currently not engaged in any other primary mental health care services and are at risk of hospitalisation as a result of their mental health condition.
Referrals are by a GP, via a provisional referral through PHN non-GP approved referrers (e.g., day-to- day living centres, community mental health) or by a Connect for Wellness Peer Worker. A GP mental health treatment plan is required following the first three sessions.
