Excess AstraZenenca
The Department of Health is working with the Vaccine Operations Centre (VOC) and logistics partners to develop a process to collect some of the excess doses of AstraZeneca. Complete this form even if you have already notified SWSPHN.

Requests are processed weekly for implementation in the following ordering cycle. SWSPHN will collate and send your responses to the COVID-19 taskforce no later than 2pm each Wednesday.
