Connect for Wellness
Psychological support program that features allied mental health professionals, peer support and group therapy for people living with severe and persistent mental illness.
Eligibility: People living with severe and persistent mental health issues who are currently not engaged in any other primary mental health care services, and are at risk of hospitalisation as a result of their mental health condition.
Services available: Access to an allied mental health professional, group therapy, and tailored assistance via a peer mental health worker, as a vital and consistent form of support throughout the program.
Referral Pathway: Referral by GP or provisional referral through PHN non-GP approved referrers (e.g., day to day living centres, community mental health) or by Connect for Wellness Peer Worker. A GP mental health treatment plan is required following the first 3 sessions.
Referral forms can be found under Health Professionals > Practice Support > Forms and Templates. GPs can access Connect for Wellness through the Mental Health Central Intake number, and fax.
