Outline
The 5As Clinical Audit aims to implement the 5As model for the prevention of chronic diseases in General Practice.
The audit focuses on monitoring and improving the recording of risk factors for your patients.
The RACGP approved clinical audit can assist practices in meeting accreditation standards for risk factor recording and aims to improve practice systems to maintain these high levels.
What’s involved?
The 5As Clinical Audit involves a six month auditing period (5As Outline)
- Baseline data collection and reflection on data
- 5As training and action plan development
- Monthly follow ups and data extractions with two-monthly reports
- Evaluation and CPD point distribution
The clinical audit uses PENCS software to record de-identified patient data to produce reports. See our 5As Resources section for Privacy Policies and more information.
